Free Account Setup Instructions

1. Go to: www.FORMED.org
2. Click on “SIGN UP” button in the middle of the page
3. Click on “SIGN UP AS A PARISHIONER” (if you already have a login, click SIGN IN)
4. In the search box type “76234” – and our church should appear
   1. Once Assumption of the ... appears click “NEXT”
5. Type your NAME and EMAIL  and then click “SIGN UP”
